Once a hot mess, always a hot mess!

Former Boardwalk Empire bad girl Paz de la Huerta has maintained a relatively low profile lately, but showed up on the red carpet of a Miami event on Wednesday night up to her old antics and exhibiting bizarre behavior, and Breaking Celebrity News has photos and exclusive eyewitness details.

Wearing a skintight black top with a purple tube skirt with blood red lipstick painted outside the lines of her lips, the actress struck a variety of provocative poses at the VIP grand opening of the H&M Lincoln Road store, where the exclusive guest list included Flo Rida and Chase Crawford, but it was inside the event that Paz really garnered attention.

"She did not at all look like she was in her right mind," an onlooker exclusively told Breaking Celebrity News. "People were making snickering comments about her all night."

And it wasn’t just her behavior that had people talking! At some point in the evening, she suffered an extremely embarrassing wardrobe malfunction, and didn't seem to notice even though everyone else did.

"On top of her somewhat dazed demeanor, she had a very noticeable hole in the back of her skirt."
